Coaching Set Up Packet




Welcome to HYBRID ATHLETE TRAINING!! I want to be the first to congratulate you on taking the next step to becoming the healthiest, BEST version of YOU. I look forward to working with you and being a part of your journey... YOU are the best project that YOU will ever work on!! Please take a few minutes to fill out the forms below so that we can best serve your needs and help you achieve your goals. 

1.Your goal worksheet 

Spend some time thinking about what your specific goals are. Your goals need to inspire you.. losing/ gaining 20lbs isn't inspiring- it's the person you are, 20 lbs lighter/heavier that is going to inspire you to keep moving forward. What does that version of you look like, and how does it feel? What activities are you doing and what does your life look like.


2. Nutritional Coaching Questionnaire

 This will give us the information that we need to set up the best nutrition plan for you. 


3. Behavior Change form

Take a look from the outside looking in on your day to day routines. What are some different behaviors that you are noticing repeatedly that you feel like are holding you back. Again, be as honest as possible so that we can create practical strategies to overcome.


4. Nutrition Set Up

This will help me get a better understanding of your current habits and patterns around nutrition so I can best help guide you to getting where you need to be in order to achieve your goal.


 5. Set up/ download app

Once you register, you will receive a link in your email to download and set up your profile in the Hybrid Athlete Training APP. Please complete the setup process and enter your current weight, age and height into the provided spaces as well as get your FITBIT set up and synced. You can choose from one of the 2 options below. Be sure to have your profile set up prior to our consultation so that I can go through it with you and upload your first program. 




6. Schedule your consultation/ Assessment

This is a 30 minute consultation call to get a better picture of your needs, your goals and your background as well as any restrictions to help you get the best possible guidance and ensure that you have a proper road map moving forward.

Please be sure to be on time, have any questions ready ahead of time and be sure you're in a quiet space. We will be covering personal topics.
